Your initial appointment will consist of a consultation. We will review your medical and dental history as well as collect clinical information necessary to diagnose the treatment you need. This clinical information may include a complete periodontal charting, x-rays and photographs.
Please assist us at the time of your initial visit to the office by providing the following information:
- Your referral slip and x-rays from your referring dentist.
- A list of medications you are currently taking, including aspirin products (Excedrin, Anacin, Bayer, Motrin and Tylenol are okay), and allergies to any medications.
- Also, please let us know ahead of time if you have to take any pre-medication antibiotics. Often if one has a history of heart murmur, rheumatic fever, pacemaker, heart attack, or artificial joints, metal plates or pins in the fingers, knees, or toes, one should check with their physician to see if they recommend antibiotics to prevent infection in that area, as even a periodontal examination or dental cleaning can stir up enough bacteria to cause very serious consequences to your overall health.
- If you have dental insurance, please bring any forms or insurance cards with you to the appointment.
Please Note: All patients under the age of 18 must be accompanied by a parent or guardian at the consultation appointment.
If your referring dentist has taken x-rays, you may request that they be forwarded to us.
